Conventional Laryngoscope Blade Patterns - Choosing the Right Configuration
The three main conventional blade patterns - Macintosh, Miller, and McCoy - are used in different clinical situations, and understanding the practical differences helps in procurement decisions.
Miller Blades - Straight Pattern for Specific Indications
Miller blades are straight rather than curved. Rather than lifting the epiglottis indirectly by pressing into the vallecula, the Miller technique involves placing the blade tip under the epiglottis and lifting it directly. This gives better glottic exposure in some patients - particularly neonates, infants, and adults with a long floppy epiglottis where the curved blade's indirect action doesn't adequately elevate it.
Miller blades also create a different tongue displacement geometry from Macintosh blades, and some anaesthetists prefer them for patients with a short mouth opening or anteriorly positioned larynx where the Macintosh technique creates less space. The Miller pattern is more common in paediatric anaesthetic practice than adult, but experienced anaesthetists in adult practice often carry Miller blades for specific difficult airways.

When the lever is depressed during laryngoscopy, the blade tip flexes upward, which lifts the epiglottis and improves the Cormack-Lehane grade without requiring additional force on the laryngoscope handle. The practical benefit is most pronounced in patients with limited cervical spine mobility, where increasing the force on a conventional blade to improve the view is either mechanically ineffective or unsafe.
The McCoy has been shown in multiple studies to improve glottic visualisation compared to a standard Macintosh in patients with poor laryngoscopy grades, which is why difficult airway trolleys and departmental protocols specifically include it.
