Elevators - Periosteal and Septum Applications

Periosteal elevation separates the fibrous layer attached to bone from the underlying cortical surface. The plane between periosteum and cortex is a true anatomical layer and, when elevated correctly, it separates cleanly without tearing. The elevator blade needs to be stiff enough to advance against the periosteal attachment without deflecting, and the edge needs to be blunt enough not to cut into the cortex or perforate the periosteum.

Cottle Septum Elevator

Maurice Cottle's approach to septal surgery was built around clean mucoperichondrial elevation as the foundation of the entire procedure - and the Cottle septum elevator reflects this. The blade profile suits the push-and-peel action of elevating adherent mucoperichondrium from septal cartilage and bone, where the instrument must advance along the correct anatomical plane without tearing through into the nasal cavity.

Freer Elevator - Double-Ended Versatility

The Freer elevator is double-ended, with a sharp edge on one face and a blunt edge on the other - the sharp edge initiates the periosteal plane where the attachment is dense, and the blunt edge continues the elevation once the plane is established. This two-in-one configuration means fewer instrument changes during a procedure where elevation sequencing changes repeatedly. The Freer appears in rhinoplasty, septoplasty, skull base approaches, and anywhere that periosteum or mucoperichondrium needs systematic elevation.

Dissectors - Blunt Tissue Plane Development

A dissector develops tissue planes by blunt separation rather than sharp cutting - pushing tissue apart along natural fascial and anatomical planes rather than dividing it. This technique minimises bleeding, avoids inadvertent injury to structures that lie in or near the plane, and creates a cleaner operative field than sharp dissection in planes where the anatomy is well defined.

Bone Curettes - Cavity Cleaning and Tissue Removal

A curette removes material from a cavity or surface by scraping - the cup at the instrument's working end engages the surface and the scraping motion brings loose material, diseased tissue, or granulation out of the cavity. In orthopaedic surgery this is used to clean out cyst cavities, bone tumour beds, and infected bone spaces. In spinal surgery, disc material is removed from the intervertebral space before fusion. The cup size and shaft length determine which site and depth the curette can reach.
