A bayonet shank exists for one reason: to stop the surgeon’s own hand blocking the view. In a nasal cavity, an ear canal or a deep neurosurgical corridor, the surgeon looks straight down the axis the instrument is working along — and an in-line forceps puts fingers, knuckles and shaft directly in that sightline. Stepping the handle sideways clears it while the tips stay on the axis.
The offset does a second job in a narrow corridor: it keeps the hand from fouling the walls of the opening. In a nostril or a speculum the working space is barely wider than the instrument, so an in-line grip has to be held partly inside the opening. A bayonet lets the hand sit outside and below while only the slim tips enter. 19 cm carries that reach to the depth these approaches require.
