Those proportions are what make a blade work inside a nostril. 46 mm of length holds a good stretch of the nasal sidewall, while 5 mm of width takes up almost none of the space it just created. A wider blade holds marginally more tissue and loses far more room, which in a cavity this size is the wrong trade.

152 mm places the working end well down the nasal cavity with the hand outside the nostril. The blade bears against mucosa for the length of the procedure, so surface finish matters as much as shape — any roughness abrades tissue that is already being handled repeatedly through a small opening.
