Tessier Nasal Osteotome, 18.5 cm – Straight, 5 mm

A 5 mm tip is narrow for an osteotome, and nasal bone is why. The cuts in rhinoplasty are made in thin bone a few millimetres from skin that will show every irregularity, so the osteotome has to remove a precise line rather than a broad swathe. A wider tip takes more bone than the cut needs and leaves a step that is visible afterwards.

Straight rather than curved means the cut runs where the handle points — the surgeon aims the line rather than following a curve the instrument imposes. At 18.5 cm the osteotome is long enough to reach along the nasal sidewall from the entry point while the hand stays outside, which matters because osteotomes are struck with a mallet and the hand holding one needs to be clear of the field.
