Self-retaining means the exposure survives the surgeon’s attention moving elsewhere. Once set, the mechanism holds the wound at a fixed opening for the length of the procedure — no assistant, no drift, and both of the surgeon’s hands free. In a single-operator field that is often the difference between a procedure being practical and needing a second pair of hands.

The left designation is not a preference. A blade shaped for the left side of an approach meets the right side at the wrong angle, contacting on a corner instead of seating flat, and a self-retaining instrument that is not seated properly works loose under its own tension. A solid blade rather than a pronged one holds continuous tissue back as a single face, which suits wound edges rather than structures needing gaps to see between.
