The pelvis is the hardest cavity to expose. It is deep, narrow at the entrance, bounded by bone that cannot be moved, and the structures inside it are mobile and easily damaged. A retractor working there has to hold tissue back across a long reach while fitting through an opening that the anatomy fixes.
The polished finish matters more here than on a surface instrument. The blade is in contact with bowel and peritoneum for the length of the procedure, and a surface with any roughness abrades tissue every time the retractor is repositioned — which in a pelvic case happens repeatedly as the exposure develops. A smooth finish also releases cleanly rather than dragging on withdrawal.
