ALM Self-Retaining Retractor – 4×4 Prongs

Self-retaining means the retractor is set once and then forgotten. In a small field there is often no room for an assistant’s hand at all, and even where there is, a held retractor drifts as attention moves elsewhere. A ratcheted frame applies the same tension from the first minute to the last — and more practically, it lets a single operator work a procedure that would otherwise need two people.

Four prongs on each side spread the hold across eight points rather than concentrating it. That matters more on a self-retaining instrument than a hand-held one, because the tension sits on the wound edge continuously for the length of the case with nobody monitoring it. Sharp prongs grip skin and fascia that would slip from blunt ones; blunt prongs press rather than penetrate, which is the right choice where the edge must not be marked.
