Adson Dura and Nerve Hook, 20 cm – 3 mm Tip

The sharp-or-blunt choice is the decision this instrument forces before the case. Dura is a membrane that can be engaged at its cut edge and lifted; a nerve cannot be engaged at all, only displaced. A sharp tip holds dura securely where a blunt one slides off it — and the same sharp tip against a nerve does damage that does not recover. Neither configuration is safe for both, which is why both are stocked.

3.0 mm is small enough to work between structures that sit close together, which is the situation in most of a craniotomy or a spinal exposure. At 20 cm the shaft carries that fine tip to the bottom of a deep approach with the hand above the wound edge rather than inside it — essential when the corridor is narrow and the surgeon is looking straight down it.
