Von Graefe Muscle Hook, 13.5 cm – 10.5 mm Hook

An extraocular muscle is a flat band attached to the globe, and the only way to handle one is to pass a hook behind it and lift. There is nothing to grasp — forceps on a muscle crush it, and the muscle has to survive being repositioned and resutured. The hook goes under, takes the whole belly on its curve, and lifts.

10.5 mm sets how much muscle the curve can hold. Too small and the hook catches only part of the belly, so the muscle slips off mid-manoeuvre; too large and it cannot pass behind the muscle in the confined orbit. At 13.5 cm the instrument is finger-held, which is what strabismus work requires when the structures are millimetres apart.
