Blunt is the specification that matters here. The anterior nasal spine sits at the base of the nose with mucosa, cartilage and the columella immediately around it, and a sharp or edged retractor working in that space perforates the very structures the procedure is trying to preserve. A blunt working end displaces them instead.

19 cm places it between the 23.5 cm bridge retractor and the shorter nasal patterns — the spine sits lower and closer to the entry than the dorsum, so less reach is needed. The instrument is often held while osteotomies are cut at the base of the nose, which is exactly when a blunt end is doing its second job: standing between the saw and the soft tissue behind it.
