A one-millimetre difference between ends sounds trivial and is not. In a small exposure the right blade is the one that holds the tissue edge without also filling the space behind it, and that margin is often a millimetre. Having both sizes on one instrument means the surgeon tries one, reverses, and tries the other without reaching for anything.
The S-curve is what gets either blade to the tissue. A straight blade approaches along the line of the handle, so the hand ends up over the field; curving the shaft twice brings the blade back onto the working plane while the grip stays off to the side. At 114 mm the whole instrument stays finger-controlled.
