The two ends are matched to two different risks. The 4 mm sharp end elevates periosteum and perichondrium along the nasal dorsum, where the tissue is firmly attached and a sharp edge is needed to find the plane at all. The 3 mm blunt round end is used where that same sharpness would be a liability — under the bony dorsum at the upper lateral cartilages, whose attachment to the nasal bones is exactly what holds the roof of the nose closed.

Separating that junction inadvertently produces an open roof deformity, one of the more visible complications in rhinoplasty and one that has to be corrected rather than left. A blunt, rounded tip pushes tissue aside along the plane instead of dividing it, so the dissection can proceed right up to the junction without cutting through it. Having both ends on one 20 cm instrument means the surgeon changes technique by reversing the handle, at the point in the dissection where the anatomy changes.
