Why ENT instruments look the way they do

Length and angle. Instruments must reach the working point while the surgeon's hand and the instrument body stay clear of the line of sight. Bayonet shafts and angled tips exist for exactly this reason, not for aesthetics.

Fine working ends. The structures involved — ossicles, septal cartilage, nasal mucosa — are millimetres across, so the tips are proportionally smaller than general instruments while the shanks stay long.

Tip-hinged action. Crocodile and alligator forceps open at the tip while the shaft stays still, which is the only practical way to grasp inside a canal a few millimetres wide.
