The teeth face inward, and that is the whole design. Tailoring a breast pocket after the implant is in means holding implant and tissue apart and moving them in opposite directions — so one face of the blade works against the implant shell and the other against soft tissue. Putting atraumatic teeth only on the inside grips the tissue while leaving the shell side smooth.
What that saves is removing the implant to adjust the pocket. Semi-malleable adds to the same purpose: the blade is bent to the pocket’s actual shape rather than the pocket being forced to the blade, and holds that shape under load without springing back. At 31.5 cm it reaches the far edge of the pocket from a small incision.
